There could be many reasons, why he did like that 1) He didn't want the old lady to know that he was helping her i.e (helping silently without showing-off) 2) If the old lady was a proud person, she would not have accepted the help 3) The funny one, he was a doctor, so he wanted the old lady to get some exercise daily, so "One stone Two birds"-VJ
